- You must physically submit to the Parks and Recreation Office for examination one document from List A OR a combination of one document from List B and one document from List C.
Required - 18 years and Older ONLY
Required - 15 years and Younger ONLY
- You must pick up the completed work permit letter from the Parks & Recreation Office
